SERVICE DESCRIPTION
Provides psychiatric assessment and consultation to adults and teenagers 17 years of age and older with an intellectual disability and a mental health concern and / or significant behavioural problem, see below for criteria.

Developmental Disabilities Mental Health (DDMH) – Arnika offers psychiatric assessment and stabilization, via medication, for individuals with Intellectual Developmental Disabilities (IDD), mental health concerns, and / or significant behavioural problems. DDMH only accepts referrals for persons who have been deemed eligible for supports with Persons with Developmental Disabilities (PDD). Once the initial assessment by the psychiatrist is complete, the patient may be referred internally for evidence based interventions provided by allied health resources internal to DDMH - Arnika. This is at the psychiatrists discretion. DDMH is a consultation service and once the patient has achieved mental health stability, a discharge back to the family physician will occur.

ELIGIBILITY REQUIREMENTS
The Arnika Centre is appropriate for individuals who:
  • Are deemed eligible for support with PDD (have applied and received a letter of eligibility from PDD)
  • Are over the age of 17 years
  • Have a family doctor and the family doctor is supportive of the referral
  • Have symptoms of a mental illness
  • May have severe behaviour problem(s) that markedly disrupts or impairs daily functioning or the client’s access to disability/community supports (ie. physical aggression, property destruction, self-injury, sexual misconduct, bolting)
    • If behaviour problems are the presenting reason for referral:
      • community- based behaviour consultation supports have been tried and failed to address the behaviour problem or
      • there are safety concerns for family, support staff, community at large;
    • Are in agreement to attend at the Arnika Centre in person for all psychiatry appointments. If living outside of Calgary city limits, understand that may not be eligible for internal allied health resources or services may be limited to psychiatry only. No CTO management outside of Calgary CTO office
    • Client/legal guardian(s) understands and agrees to medication as the primary focus of treatment;
    • Are unable to participate in services at traditional community mental health clinics due to disability needs or behavioural challenges.
The Arnika Centre is not appropriate for individuals who/whose
  • are able to have needs met through traditional community mental health services or
  • disability support needs are unmet by PDD and behavioural challenges are largely related to unmet needs for supervision or support;
  • primary problem, in addition to IDD, is borderline personality disorder and the individual is not amenable to change oriented therapy.
  • primary problem, in addition to the IDD, is substance use disorder and the client is resistant to SUD treatment;
  • referral is for court mandated treatment (please note forensic history is not an exclusion criteria)
  • has a history of being discharged from the Arnika Centre for lack of engagement or due to extreme targeted threats or violence to staff or property destruction

The Arnika Centre is not connected to any residential facilities or disability support programming. For such services, please contact the patient's PDD caser worker. Arnika does not provide crisis services, community outreach, crisis services or case management.
The following are criteria for discharge from the Arnika Centre:
  • Presenting mental health symptoms and concerns have been explored fully and stabilized as much as possible or there have been no medication changes for at least one year, and there has been no recurrence or worsening of presenting concerns during.
  • The patient’s outstanding difficulties cannot be directly attributed to modifiable psychiatric disorder (are behavioural in nature or related to severe and chronic personality disorder or severe substance use disorder)
  • Patient reports ongoing mental health symptoms but is disinterested in change therapy (such as prescribed medication, changes to lifestyle or evidence-based psychotherapy as recommended by psychiatry)
  • The medication regimen can be reasonably supported by a primary care physician
  • Failure to attend regularly scheduled appointments

In this scenario the captured referral guidelines in ARD can become "orphaned" as they are not attached to any current IA healthcare service.

Categories of non-current or orphaned referral guidelines: INDIVIDUAL and COMMON.
The REFERRAL GUIDELINES section of the profile has the prefix INDIVIDUAL or COMMON to help you choose the method below when transferring referral guidelines from a non-current Healthcare Service@Location to a current healthcare service@location.

Individual referral process
  1. Click on a non-current (Red) Healthcare Service@Location at the bottom of the All Locations list.
  2. The non-current referral info is displayed with the link Copy this Referral Process to another Healthcare Service@Location link on the upper right hand corner. Click on the copy link.
  3. Choose a current location (Green or Brown) from the All Locations list. This will be the Healthcare Service@Location you are pasting the referral info into.
  4. The system will display the Edit Referral Info screen populated with the referral info from the non-current Healthcare Service@Location you viewed in the first step.
  5. Click Save and the referral info is transferred from the non-current Healthcare Service@Location to the current one.
  6. Repeat these steps for each Healthcare Service@Location that needs attention.

Common referral process - 2 sub cases.
Case 1: At least 1 current Healthcare Service@Location with common referral info is with current status for this healthcare service; One or more Healthcare Healthcare Service@Locations where replaced by new one.
  1. Click on any current Healthcare Service@Location whether it has referral info (Green) or not (Brown).
  2. The healthcare service location opens in the Edit Referral Info screen populated with the current common referral info.
  3. Save it. 
  4. All locations will be updated with the common referral information, including all the locations that don't have referral info yet (Brown). The non-current referrals (Red) will also be updated.
Case 2:  All Healthcare Healthcare Service@Locations for a healthcare service are set to a non-current status and replaced by new ones. In this case there is no current additional referral info to copy from, so the only alternative is to pick up the non-current common referral process (Red). Follow the steps described in the section Individual Referral Process above to copy/paste the non-current common referral info to the current healthcare service locations.
Generally we want to replicate current common referral info to new or replaced healthcare service locations. We only resort to copying non-current common referral info if there is no other option.

Remember: Some fields can be location specific with the common referral process:
Parking Instructions, Directions, Parking Map, Wait Time, Referral Phone or Referral Fax.
To update these items you have to edit each Healthcare Service@Location separately.

ADDITONAL NOTES:
  • The info icon after the All Locations drop down will be visible to ARD Administrators.
  • The system doesn't allow you to copy referral information from one non-current Healthcare Service@Location to another.
