SERVICE DESCRIPTION
This program specializes in managing swallowing and airway problems.This is a multidisciplinary program that manages infants and children who may have an airway and swallowing disorders.

Services that this clinic may offer include:
  • Structured clinical assessment
  • Instrumental tests of swallowing called Fiberoptic Endoscopic Evaluation of the Swallow (FEES) or Video Fluoroscopic Swallowing Study (VFSS - a.k.a. Modified Barium Swallow or MBS), Impedance testing to evaluate a child's ability to eat and swallow safely and comfortably
  • Investigations into any associated breathing awake or asleep including sleep studies, airway or gastro-intestinal endoscopies.
  • Management options which may include medical treatment, modification of feeding (e.g. changing the thickness of liquids or type of nipple or bottle, surgical need for further investigations or the need for involvement of other specialists
There are two tiers for the program
  • The Aspiration Clinic where the assessment is managed by the Speech Language Pathologist and the Otolaryngologist. For more information visit our website.
  • The Aerodigestive Clinic where a Pediatric Respirologist and Gastroenterologist joining the other members to provide their services. Additionally Dieticians and Occpation Therapy services are often sought and their members join the team
  • The program works closely with Home Nutrition Support Program.

A legal guardian must be present during your consultation.
Children, toddlers and infants under 17 years of age who are not under the care of The Cleft Lip and Palate Clinic or the Glenrose Rehabilitation Hospital Feeding and Swallowing Clinic - except by direct consultation of either team.
The profile of patients may include:
  1. Clinical Problems: chronic stridor (>4 weeks in the absence of infective symptoms), increased WOB, recurrent chest infections, blue spells, apparent life-threatening episodes, daily chocking during feeding, chronic cough.
  2. Background of ex-prematurity at birth, intubation at birth, Gastroesophageal reflux disease, suspicion of chronic lung disease or pre-school asthma should increase suspicion
  3. Two years of age and younger take priority and is the commonest age of presentation
The Aerodigestive clinic remains a higher tier of referral that is reserved for the judgment of the aerodigestive team members.
Common findings and conditions managed primarily or secondarily:
  1. Laryngomalacia
  2. Laryngeal mobility disorders
  3. Subglottic stenosis
  4. Tracheal and bronchiole stenosis and malaria
  5. Obstructive sleep apnea
The programs collaborate with the Tracheal surgery, Pediatric Cardiovascular and Home nutrition Support Programs at the Stollery Children’s Hospital. PATIENT APPOINTMENT INSTRUCTIONS
A parent or legal guardian must attend the appointment with the child, if this is not possible on the date you have been assigned call the office to reschedule to an appropriate date.
You will need to bring the patient’s valid Alberta Healthcare card with you.
If the patient is on any medications, nasal sprays or ear drops – make sure you know what they are and how much is being used.

If further involvement from other specialists is needed the child is invited to the Aerodigestive Clinic where a Pediatric Respirologist and Gastroenterologist provide their services.

What to Expect - For Aspiration Clinic, Pediatric
A clinic visit may take approximately 30 minutes and your child might be seen by a physician and a speech language pathologist. The purpose of the clinic is to assess patients with possible airway issues as well as feeding and swallowing issues.
The care team may propose to use a telescope to look into the nose and throat of your child. This does not require him/her to be asleep.
  • Make sure your child does NOT eat or drink 1 ½ - 2 hours before this appointment, as a child who is hungry is more likely to drink during this visit.  All medications should be taken at regularly scheduled times.
  • Bring your child’s feeding utensils (favorite bottles/nipples, pacifiers, cups etc.). Depending on your child’s age and feeding experiences, bring along something for your child to drink. If you are currently adding thickener to your child’s drinks, bring something to drink at the current consistency that your child is drinking, as well as something that is UNTHICKENED. Arrive with your child’s drinks prepared (mixed, warmed up if needed, etc.) and ready to drink. 
Interpreters speaking many different languages are available to assist you through our multicultural program. Should you require language interpreter services for your child’s clinic visit, please schedule this service in advance so an interpreter can attend with you.
It is imperative that a parent or guardian attends this appointment with the patient.

In this scenario the captured referral guidelines in ARD can become "orphaned" as they are not attached to any current IA healthcare service.

Categories of non-current or orphaned referral guidelines: INDIVIDUAL and COMMON.
The REFERRAL GUIDELINES section of the profile has the prefix INDIVIDUAL or COMMON to help you choose the method below when transferring referral guidelines from a non-current Healthcare Service@Location to a current healthcare service@location.

Individual referral process
  1. Click on a non-current (Red) Healthcare Service@Location at the bottom of the All Locations list.
  2. The non-current referral info is displayed with the link Copy this Referral Process to another Healthcare Service@Location link on the upper right hand corner. Click on the copy link.
  3. Choose a current location (Green or Brown) from the All Locations list. This will be the Healthcare Service@Location you are pasting the referral info into.
  4. The system will display the Edit Referral Info screen populated with the referral info from the non-current Healthcare Service@Location you viewed in the first step.
  5. Click Save and the referral info is transferred from the non-current Healthcare Service@Location to the current one.
  6. Repeat these steps for each Healthcare Service@Location that needs attention.

Common referral process - 2 sub cases.
Case 1: At least 1 current Healthcare Service@Location with common referral info is with current status for this healthcare service; One or more Healthcare Healthcare Service@Locations where replaced by new one.
  1. Click on any current Healthcare Service@Location whether it has referral info (Green) or not (Brown).
  2. The healthcare service location opens in the Edit Referral Info screen populated with the current common referral info.
  3. Save it. 
  4. All locations will be updated with the common referral information, including all the locations that don't have referral info yet (Brown). The non-current referrals (Red) will also be updated.
Case 2:  All Healthcare Healthcare Service@Locations for a healthcare service are set to a non-current status and replaced by new ones. In this case there is no current additional referral info to copy from, so the only alternative is to pick up the non-current common referral process (Red). Follow the steps described in the section Individual Referral Process above to copy/paste the non-current common referral info to the current healthcare service locations.
Generally we want to replicate current common referral info to new or replaced healthcare service locations. We only resort to copying non-current common referral info if there is no other option.

Remember: Some fields can be location specific with the common referral process:
Parking Instructions, Directions, Parking Map, Wait Time, Referral Phone or Referral Fax.
To update these items you have to edit each Healthcare Service@Location separately.

ADDITONAL NOTES:
  • The info icon after the All Locations drop down will be visible to ARD Administrators.
  • The system doesn't allow you to copy referral information from one non-current Healthcare Service@Location to another.
