Rehabilitation Criteria
Requires nursing care, access to daily physician/alternate assessment, two or more interdisciplinary rehabilitation interventions (e.g., OT, PT, Nursing, SW, SLP, Psychology).
Rehabilitation and/or care needs cannot be met in the community.
Demonstrates the ability to tolerate, retain information, and actively participate in intensive interdisciplinary rehabilitation from a physical, cognitive, and mental health perspective.
Able to participate in high intensity rehabilitation for up to 3 hours per day, 7 days per week, with the tolerance to remain out of bed and seated for 1–2 hour intervals.
Demonstrates consistent motivation and willingness to participate in all areas of
rehabilitation which may include group classes, 1:1 sessions, and self-care.
Identifies goals for rehabilitation that are specific, measurable, attainable, realistic, and time-limited, which cannot otherwise be met in the community. Individualized functional goals are collaboratively developed with the patient, family, and interdisciplinary team.
Medical Criteria
Medically stable, comorbidities have been established; there are no undetermined medical issues; vital signs and laboratory values are within normal limits or stable at the patient’s baseline. There is an established medical plan of care, and the referring team has determined that the patient is stable for rehabilitation.
Maintains adequate oxygen saturation during rehabilitation without increased shortness of breath.
Stable wounds requiring more than 1 hour of daily nursing care will be considered case-by-case; wounds must not interfere with the ability to sit, weight-bear, or participate in rehabilitation. Recent wound photos must be available in Connect Care for review.
Life expectancy must be greater than 12 months.
Pain is well controlled and does not interfere with participation in rehabilitation.
Cancer treatments may be considered if they do not interfere with rehabilitation, participation, or cause significant fatigue or functional limitation.
Tracheostomies, LVADs, and bolus tube feeds will be considered case-by-case, with primary emphasis on the patient’s ability to participate in physical rehabilitation.
Other Criteria
Service area primarily includes Northern Alberta, Northern British Columbia, and the Northwest Territories. Patients must have valid provincial healthcare coverage or insurance.
Patients from outside the Edmonton corridor who cannot be discharged directly home after completing rehabilitation will be repatriated to their home corridor.
Acknowledgement that admission to Glenrose Rehabilitation Hospital is short-term and goal-focused, with discharge planning for home/community initiated at admission.
The program and admission criteria are discussed with the patient and family, and an information handout is provided. Patient agreement and understanding are documented in the medical record.
Patients must be psychiatrically stable and free of certifications or conditions requiring follow-up beyond what can be managed by MRP/Physiatry. (Note: Glenrose does not provide adult psychiatry services.)
Some patients may meet criteria for multiple inpatient programs; the intake team will determine the most appropriate placement.
In certain cases, patients may be admitted to an “off-service” bed while still receiving the required rehabilitation and support to progress toward their goals and discharge plan.
