MISSED APPOINTMENT GUIDELINES
After three missed, without appropriate reasoning a new referral will be required.
PATIENT APPOINTMENT INSTRUCTIONS
- Bring your Alberta health care card and a piece of government issued photo ID.
- Check in at reception 15 minutes prior to your scheduled appointment time.
- Please make sure to have someone with you if you are unable to communicate in English.
