MISSED APPOINTMENT GUIDELINES
If a patient has missed two scheduled appointments a new referral from their healthcare provider is required.
PATIENT APPOINTMENT INSTRUCTIONS
- Bring your Alberta Health Care card and a piece of government issued photo ID.
- Check in at reception 15 minutes prior to your scheduled appointment time.
- Do not attend the appointment if you or your child is ill; please cancel as it may impact test results. Call 1-888-925-4044 (Central Intake) to reschedule.
- You may bring a family member or significant other during your consultation.
- Do not bring additional children to the appointment unless another adult can be present with them; children cannot be left unattended while parent is involved in testing.
- An interpreter services are available on request.
