• A late cancellation is when you call the day of your appointment or group session to cancel.
• a No-Show appointment will be discussed, exploring barriers to your attendance. If solutions cannot be found after 2 No-Shows, your team may discharge you from the program.

• Bring your Alberta health care card and a piece of government issued photo ID.
• Check in at reception 15 minutes prior to your scheduled appointment time.
• You may bring a family member or significant other during your consultation.
• Please make sure to have someone with you if you are unable to communicate in English.
