MISSED APPOINTMENT GUIDELINES
Patients will receive an automated phone call from Alberta Health Services (AHS) and be asked to confirm their appointment. Confirming the appointment will notify the clinic that you will be able to attend the appointment. If an appointment is not confirmed, the patient is required to rescheduled.

If you are unable to attend a scheduled appointment, please contact the clinic a minimum of 48 hours in advance. If a patient misses an appointment, they will be called to reschedule.

Missed appointments are minimized by requiring a re-referral after two missed appointments.
PATIENT APPOINTMENT INSTRUCTIONS
  • Bring your Alberta health care card and a piece of government issued photo ID.
  • Check in at reception 15 minutes prior to your scheduled appointment time.
  • You may bring a family member or significant other during your consultation.
  • Please make sure to have someone with you if you are unable to communicate in English.
