MISSED APPOINTMENT GUIDELINES
If the patient needs to cancel or reschedule their appointment we ask for 24 hours notice. If the patient fails to give advanced notice(no-show):
- New referral appointment will not be rescheduled without a re-referral
- Two (2) or more no-shows and/or rescheduled appointments may result in dismissal from our practice
PATIENT APPOINTMENT INSTRUCTIONS
Patients should be instructed to arrive for their appointment 30 minutes prior for parking and check-in. They need to bring:
- Alberta Health Care Card
- Photo ID
- Family member or close friend
- List of current medications, including any homeopathic supplements
- Reading glasses, or any other visual aids, if applicable
- Walking aids, for example a cane or a walker, if applicable
- Hearing aids, if applicable
