MISSED APPOINTMENT GUIDELINES
If you are unable to attend an appointment, at least 4 hours notice must be given. If you miss one appointment, by cancellation without 4 hours notification or failure to attend) you will be automatically discharged. You will receive a letter indicating same.

- Bring your Alberta health care card and a piece of government issued photo ID.
- Check in at reception 15 minutes prior to your scheduled appointment time.
- You may bring a family member or significant other during your consultation.
