MISSED APPOINTMENT GUIDELINES
Clients will be discharged from our clinic if:
- They have not responded to 2 scheduled telephone follow-up appointments.
- They have 2 documented no shows.
- They cancel their appointment 2 consecutive times without unforeseen or extenuating circumstance.
PATIENT APPOINTMENT INSTRUCTIONS
- Bring your Alberta health care card and a piece of government issued photo ID.
- Please make sure to have someone with you if you are unable to communicate in English.
- If your appointment is 9PM or later, you must come in through the emergency department entrance.
- Have something to eat and be well hydrated.
NOTE: Eating and drinking in the clinic is not allowed unless medically necessary.
