If a new patient misses two appointments, the referral is closed and the referring physician is notified. Please ensure all patient demographics especially telephone number are current.

- Bring your Alberta health care card and a piece of government issued photo ID.
- Check in at reception 15 minutes prior to your scheduled appointment time.
- You may bring a family member or significant other during your consultation.
- Please make sure to have someone with you if you are unable to communicate in English.

DIRECTIONS
The Health Sciences Centre is attached to the Foothills Medical Centre located at the intersection of 16 Ave NW and 29 St NW. When you have parked go to level 3 in the parkade and take the +15 over to the Health Sciences Centre. Once you have entered the building, turn left and follow the corridor to our reception.
PARKING INSTRUCTIONS
Park in Lot 6, pay for parking and proceed to the third level of the parkade to take the +15 overpass to the Health Sciences Centre.
