MISSED APPOINTMENT GUIDELINES
Referrals will be closed and will require a new referral when patient is unreachable due to expired contact information or unresponsive to 3 call attempts, or patient "no shows" to 2 appointments without valid reason.
If you are unable to attend a scheduled appointment, please contact the clinic a minimum of 48 hours in advance.
PATIENT APPOINTMENT INSTRUCTIONS
- Bring your Alberta health care card and a piece of government issued photo ID.
- Check in at reception 15 minutes prior to your scheduled appointment time.
- You may bring a family member or significant other during your consultation.
