MISSED APPOINTMENT GUIDELINES
If a patient misses an appointment, they will be contacted to reschedule. If a patient misses 3 appointments, care will be transferred back to the referring provider and a new referral would be required.
PATIENT APPOINTMENT INSTRUCTIONS
  • Bring your Alberta health care card and a piece of government issued photo ID.
  • Check in at reception 15 minutes prior to your scheduled appointment time.
  • You may bring a family member or significant other during your consultation.
  • Please make sure to have someone with you if you are unable to communicate in English.

DIRECTIONS
The Lethbridge Wound Clinic is located Downtown Lethbridge at the Melcor Centre. Address is Suite 110, 200 4th Ave S. Access to the clinic is through the door on the West side of the building.
PARKING INSTRUCTIONS
There is a Parking lot on the west side of the block in front of the Melcor Centre. It is 90min free parking, there are Kiosks that you will input your license plate number.
