Clients must phone the site to cancel and reschedule. After 2 missed appointments with no prior contact, the client will be discharged and will need to restart the referral process.

• Bring your Alberta health care card and a piece of government issued photo ID.
• Check in at reception 15 minutes prior to your scheduled appointment time.
• You may bring a family member or significant other during your consultation.
• Please make sure to have someone with you if you are unable to communicate in English

At the Daysland Medical Clinic, follow the hallway past reception to your left, then turn right. Door to the clinic is at the end of the hallway. It is signed. It is preferred that clients phone the clinic when they arrive and someone will come to reception and meet them.
