Patients who miss multiple appointments without notifying the clinic, may require a new referral.

- Check in at reception 15 minutes prior to your scheduled appointment time.
- You may bring a family member or significant other during your consultation.

DIRECTIONS
We are located in the Holy Cross Ambulatory Care Centre, on the North side of the Rockyview Hospital. The office is on the main floor, by the coffee stand and to the left of the elevators in the Outpatient Clinic.
