If you are unable to attend a scheduled appointment, please contact the clinic a minimum of 48 hours in advance. You may be charged a fee for an appointment cancelled with less than 48 hours notice or for a no show.
PATIENT APPOINTMENT INSTRUCTIONS
- Bring your Alberta health care card and a piece of government issued photo ID.
- Check in at reception 15 minutes prior to your scheduled appointment time.
- Please pay for parking before coming upto the office.
- You may bring a family member or significant other during your consultation.
- Please make sure to have someone with you if you are unable to communicate in English.
